Policies and services on older adults

The importance of the elderly in the world, in the present century, is indisputable. In Ecuador, people over 65 are a huge promise for society, but they lack policies related to them, both in academia and in services.

In medical schools, pediatrics is taught in undergraduate and postgraduate courses of this specialty, which is justified by the biological, psychological, cultural and social aspects of this population group. However, geriatrics (the older adult's clinic) and gerontology (the epidemiology of geriatrics) are not taught, even though their percentage of the population is increasing rapidly. And the postgraduate courses in these specialties are scarce.

But, in addition, the aforementioned should not only refer to the disease, its prevention, cure and rehabilitation, but to the healthy life, that is to health, and its promotion, of the elderly, as a priority. It is crucial that the biological, psychological, cultural and social aspects, healthy, of this population group, be introduced in the academy, as well as in the services.

In the academy teaching, research and the link with society with them, and in the services the promotion of health care, and, in case of illness, the reference from ambulatory services to hospitals for specialized diagnoses and specialized treatments on the National Health System.

For the national reference there must be at least five specialized hospitals for the elderly in Guayaquil, Cuenca, Portoviejo and Riobamba, in addition to the existing one in Quito. These services for the elderly must take care of from the familiar or domiciliary environment, in the community and territorial, to the social in general. And it must be coordinated between public institutions, MSP and IESS, as well as between specialized services (which will include palliative hospital services), with the standardized nursing homes for MIES. Thus the length of the stays, short of treble, average of intervened, or long of chronic, would be supported by diverse units distributed according to the population needs.

For advantage the government contributed to a healthy nutrition with the nutritional semaphore, and the new one proposes housing and the universal pension offered as universal rights. The Academy already contributes to continuing education for its empowerment, ethnic, gender and generational equity. This is health promotion.
